How We Can Help

Living with a chronic condition is easier with a GP who knows you and your history. All our doctors are experienced in diagnosing and managing long-term conditions, and we focus on keeping you healthy and active.

We manage asthma, diabetes, heart disease, high blood pressure, chronic pain, arthritis, thyroid conditions, blood disorders, osteoporosis, emphysema, dementia, chronic bowel conditions, cancer, and kidney and liver problems. If you’re eligible, you may qualify for a government-funded personalised care plan, including three-monthly reviews, and subsidised visits with allied health professionals. We’ll provide the referral you need to claim.

What to expect

  • Book an appointment

    Book online or call 02 8209 3999. Ask for a longer consultation if you’d like to set up a care plan.

  • Review your condition

    Your GP will review your history, symptoms, medications and how the condition affects your daily life.

  • Create your care plan

    If eligible, your GP will prepare a personalised plan with clear goals and the services that will support you.

  • Regular reviews

    We’ll check in regularly, including three-monthly reviews under your plan, and provide referrals to allied health providers.
