As we age, new health conditions can develop and need ongoing support. Our doctors are experienced in older adult care and focus on helping you stay independent and well.
We help with conditions including dementia, Parkinson’s disease, osteoporosis, arthritis, heart disease, stroke, mobility problems and falls, and hearing and vision problems. We can coordinate local services to help you remain at home, or support you in the move to assisted care. We can also continue your care in local nursing homes. If you’re over 75, you’re eligible for a free, government-funded annual health assessment.

Your GP will review your medical history, medications, mobility, nutrition and daily living.
Together you’ll agree a plan covering treatment, home safety, support services and any referrals.
We’ll arrange regular reviews, coordinate with family and carers, and help you adjust as your needs change.
